Information provided on this referral form should only be your own. This form is not to be used by a third party to provide personal information about another individual.
Purpose of collecting your information
The personal information is collected for the purpose of registering your candidate in the specified program and/or related purposes. If you do not provide some or all of the personal information requested, your interest in these positions may not be successfully registered.
Disclosure of your information
The personal information may be disclosed to third parties including but not limited to members of the Australian Retailers Association . The personal information may also be disclosed to other parties for purposes other than the purpose set out in this privacy notification and consent form where you have agreed, or where it is otherwise permitted under the Privacy Act. The personal information is unlikely to be disclosed to overseas recipients.
